When Do You Need Knee Replacement Surgery? 7 Key Signs

Knee osteoarthritis affects millions of people in India, yet many patients delay seeking treatment out of fear of surgery. The truth is: not everyone with knee arthritis needs surgery, and when surgery is needed, modern knee replacement is one of the most life-transforming procedures in medicine.

As an orthopaedic surgeon who specialises in joint replacement in Delhi, I see patients every day wondering whether they've reached the point where a knee replacement is the right decision. Here are the 7 signs I look for.

1. Severe Pain That Limits Daily Activities

The primary reason to consider knee replacement is pain that significantly limits your ability to perform daily activities – walking to the market, climbing stairs, getting up from a chair, or even sleeping. When arthritis pain is this severe and consistent, it is a strong indicator that the joint is significantly damaged.

Key question to ask yourself: "Am I modifying my life around my knee pain?" If the answer is yes, it's time to see a specialist.

2. Pain at Rest and at Night

Early arthritis typically hurts only during activity. As it progresses, pain occurs even at rest and disturbs sleep at night. Night pain is a red flag that the arthritis is severe and unlikely to be managed with physiotherapy or medication alone.

3. Failure of Conservative Treatment

Before considering surgery, most patients should have tried:

  • Regular physiotherapy for at least 6–8 weeks
  • Pain medications (NSAIDs like diclofenac or paracetamol)
  • Weight loss (if overweight)
  • Activity modification
  • Walking aids
  • Intra-articular injections (steroid or hyaluronic acid)

If all these measures have been tried and still provide inadequate relief, knee replacement should be seriously considered.

There Is No Perfect Age for Knee Replacement: The decision is based on your quality of life and the degree of joint destruction – not your age. I have performed successful knee replacements on patients ranging from their early 50s to their late 80s.

4. Significant Knee Deformity

Progressive arthritis often causes the knee to bow outward (varus deformity – "bow legs") or inward (valgus deformity – "knock knees"). Once a significant deformity develops, it places abnormal stress on other parts of the knee and accelerates joint destruction. Visible deformity that is worsening is a strong indicator for surgery.

5. X-Ray Shows Bone-on-Bone Contact

A standing X-ray (weight-bearing) is the most important investigation. I look for:

  • Loss of joint space: The gap between the bones has narrowed – indicating cartilage loss
  • Bone-on-bone contact: No remaining cartilage; the bones are touching
  • Osteophytes (bone spurs)
  • Subchondral sclerosis or cysts

When X-rays show bone-on-bone contact, conservative treatment has very limited value in the long term.

6. Knee Stiffness Lasting More Than 30 Minutes

Stiffness after rest (called "gelling") that lasts more than 30 minutes in the morning or after sitting is a sign of significant joint inflammation and damage. When stiffness is this severe, it affects the ability to function independently and is an important sign that the arthritis is advanced.

7. Worsening Quality of Life and Mental Health Impact

Chronic pain takes a toll not just physically but also mentally. Depression, social withdrawal, weight gain from inactivity, and inability to participate in family activities are all real consequences of undertreated knee arthritis. A good quality of life is a legitimate reason to consider surgery – don't wait until you're completely immobile.

What Are the Alternatives to Knee Replacement?

Depending on your age, severity of arthritis, and which compartment of the knee is affected, other options may include:

  • Partial (Unicompartmental) Knee Replacement: If only one compartment is affected
  • High Tibial Osteotomy (HTO): Realigning the leg to shift weight away from the damaged compartment – best for younger patients with early arthritis
  • Arthroscopy: Limited role in arthritis, but can help with mechanical symptoms like locking
  • PRP Injections: May provide temporary relief in early arthritis

The Bottom Line

Knee replacement is a major decision, but for the right patient at the right time, it is truly transformative. Most of my patients tell me they wish they hadn't waited as long as they did. Modern techniques, improved implants, and rapid recovery protocols mean you can be walking within 24 hours and returning to normal life within weeks.
