Hip Replacement Recovery: Complete Week-by-Week Timeline

Total hip replacement (THR) is one of the most successful and life-changing operations in medicine. Patients who have suffered months or years of hip pain, stiffness, and reduced mobility frequently tell me after surgery: "I wish I had done it earlier."

Understanding what the recovery looks like – week by week – helps patients prepare mentally and physically, and sets realistic expectations. Here is what to expect based on my experience performing hip replacements in Delhi.

Before Surgery: Preparation is Key

Good preparation significantly speeds up recovery:

  • Attend pre-operative physiotherapy to strengthen hip and leg muscles
  • Arrange your home: raised toilet seat, grab rails, non-slip mats
  • Stop smoking and optimise diabetes/blood pressure control
  • Understand the hip precautions you'll need to follow post-surgery
  • Arrange someone to help at home for the first 2–4 weeks

Day of Surgery (Day 0)

Surgery typically takes 60–90 minutes under spinal or general anaesthesia. You will be in the recovery room for 1–2 hours, then shifted to your ward room. Pain is managed with a combination of medications including local anaesthetic injections around the joint. Most patients are surprised by how comfortable they feel.

Day 1–2 After Surgery

  • The physiotherapist will get you sitting up and standing on Day 1
  • Most patients take a few steps with a walker on Day 1
  • You will be taught hip precautions: no bending the hip beyond 90 degrees, no crossing legs, no twisting
  • Pain is well-controlled with oral medications
  • Blood thinners are started to prevent clots

Rapid Recovery: At my practice, most patients are walking within 24 hours of hip replacement. The goal is to get patients home as quickly as safely possible – usually within 2–3 days for straightforward cases.

Week 1–2: Home Recovery

  • Walking with a walker around the house; short distances outdoors
  • Gradually increasing walking distance each day
  • Hip exercises as taught by physiotherapist (ankle pumps, leg slides, straight leg raises)
  • Wound check at 10–14 days; sutures or staples may be removed
  • Can manage stairs (one at a time, with support)
  • No driving yet

Week 3–6: Increasing Activity

  • Progress from walker to walking stick (usually around week 3–4)
  • Walking distance increases significantly – short walks outside
  • Light household activities resumed
  • First follow-up X-ray and clinic visit usually at week 6
  • Driving: usually cleared at 6 weeks (check with your surgeon)
  • Outpatient physiotherapy sessions begin

Month 2–3: Return to Function

  • Most patients discard the walking stick around 6₳-roving – most daily activities comfortable

Month 3–6: Full Recovery

  • Walking without any aid, at normal pace for long distances
  • Return to moderate recreational activities: cycling, swimming, golf
  • Can climb stairs normally (alternating feet)
  • Sleeping on the operated side usually permitted around 3 months
  • Final follow-up X-ray at 6 months to confirm implant position

Long-Term (Beyond 6 Months)

Modern hip implants last 20–30 years in most patients. The key to longevity is:

  • Maintaining a healthy weight
  • Avoiding high-impact activities like running and jumping
  • Regular annual or biannual follow-up with your surgeon
  • Reporting any unusual pain, click, or instability promptly

Activities to Avoid Long-Term After Hip Replacement

  • Running and jogging on hard surfaces
  • High-impact sports (squash, contact sports)
  • Extreme bending at the hip (some yoga positions)
  • Sitting on very low chairs or sofas that require excessive hip flexion

Tips to Speed Up Recovery

  • Do your physiotherapy exercises consistently – they make a huge difference
  • Keep your wound dry and clean
  • Eat a protein-rich diet to support muscle and tissue healing
  • Stay well-hydrated
  • Walk little and often – don't overdo it or under-do it
  • Attend all your follow-up appointments

Recovery after hip replacement is a journey, not a sprint. With the right surgical technique, modern implants, and a motivated patient, the results are outstanding. My patients consistently rate their satisfaction with hip replacement among the highest of any surgical procedure.
